Billing Policies & Guidelines

Your questions regarding Portland Dermatology Clinic's billing procedures and fees can be answered by our billing service Monday through Friday from 8am to 5pm at 503 223-3150. If you require special financial arrangements, please ask the billing representative to assist you.
Payment Policy
Patients are asked to bring their current insurance identification card to
each appointment. It is your responsibility to inform us of any changes in insurance, address, or telephone number.
If your insurance information is not received within 5 business days of your appointment, the balance incurred from your visit will be your responsibility and your insurance will not be billed.
HMO/PPO Plans: HMO/PPO co-payments are due at the time of each visit. You will be responsible for paying your annual deductible, copayment and charges for any non-covered services.
Medicare: We accept Medicare assignment and we will bill your secondary insurance for you. You are responsible for paying your deductible and any amount not paid by Medicare or your secondary insurance.
Commercial Plans: Patients who are covered by private, commercial plans in which our physicians are not providers will be required to pay 35% of the total bill at the time of service. The entire unpaid balance left after payment from your insurance will be billed to you regardless of the benefits and payment policies of your carrier.
Private Payment, Non-Contracted Insurance, High Deductible Plans (over $2000) and Health Savings Accounts
Initial Visit: Patients are required to pay a $138.00 down payment at the time of service. If you receive treatment that day, you will also need to pay an additional $150.00 toward the cost of that procedure.
Ongoing Services and exams: For services that are deemed medically necessary, cash payments at the time of service are eligible for a 20% discount. As stated below, for cosmetic procedures and services, full payment is due at the time of service.
Cosmetic Procedures: Payment for any cosmetic procedure is due at the time of service. You will be provided with a cost estimate prior to the procedure.
Important Note About Your Insurance Benefits
Please be sure to check your insurance benefits. Treatments and procedures recommended to you are considered by your insurance company as a separate charge from the office visit with the physician, even if the appointment occurs on the same day as your appointment with the physician. Usually, co pays and deductibles apply to these types of appointments. For specific questions regarding your insurance policy guidelines, please contact your carrier’s customer service department.
