Insurance Information

Portland Dermatology Clinic accept most insurance plans. We are Medicare participating physicians. Please contact your insurance provider prior to your appointment to ensure that your visit will be covered and that all necessary referrals are in place. We require proof of insurance at the time of your appointment, otherwise a deposit of $138.00 for the new patient visit is due in full. If you have a procedure during that visit, an additional deposit of $150.00 will be due at the time of the visit and applied to any balance owed for services provided during that visit.

Important Note About Your Insurance Benefits

Please be sure to check your insurance benefits. Treatments and procedures recommended to you are considered by your insurance company as a separate charge from the office visit with the physician, even if the appointment occurs on the same day as your appointment with the physician. Usually, co pays and deductibles apply to these types of appointments. For specific questions regarding your insurance policy guidelines, please contact your carrier's customer service department.

Referral Requirements

If your insurance plan requires a referral, you will need to request the referral from your primary care physician prior to your appointment.

Private Pay Arrangements

Out of pocket (or private) payment for dermatology services not covered by your insurance plan are due at the time of your visit. To receive the cash up front discount for medically necessary procedures, full payment is required at the time of the visit. All fees for cosmetic procedures must be paid in full at the time of service and are excluded from any discounts.
